Online Application to International Leadership Summit:
for Students, Corporations, and Community Partnerships
APPLICATION INFORMATION and REQUIREMENTS
We invite students, faculty members and community activists to participate in the first International Leadership Summit at Sias International University.
The Summit will focus on the leadership, entrepreneurship, and social responsibility skills essential to achieve each of the eight United Nations Millennium Development Goals (MDG): Eradicate Extreme Poverty and Hunger, 2) Achieve Universal Primary Education, 3) Promote Gender Equity and Empower Women,4) Reduce Child Mortality 5) Improve Material Health, 6) Combat HIV/AIDS, Malaria and Other Diseases, 7) Ensure Environmental Sustainability, and 8) Develop Global Partnerships
Students, University Faculty, and Community/Business members interested in participating must submit the application by May 15, 2011. Upon review of the application a letter of invitation will be sent to accepted applicants by May 23. Accepted participants must complete their registration by submitting payment of the registration fee by May 30.
THE COST OF THE 3-WEEK SUMMIT IS $900.00 US Dollars.
PAYMENT IS DUE IN FULL WITHIN 14 DAYS OF ACCEPTANCE IN THE PROGRAM BY CREDIT CARD (VISA OR MASTERCARD) OR CHECK PAYABLE TO GLOBAL INTERACTIONS.
This fee covers summit registration, lodging, summit academic/instructional program and site visits integral to the projects, cultural programs, and optional Mandarin Chinese instruction 3 nights each week. Not included are international and domestics travel to and from Sias campus, and items of a personal nature. The Summit will be conducted in English. Mandarin translation will be available.

General Release, Promissory Note and Hold Harmless Agreement
for Students and Faculty Participating in the International Leadership Summit (Required):
I, the undersigned, agree that I am voluntarily participating in the International Leadership Summit on Sias International University Campus organized and conducted in partnership with Sias International University, Global Interactions, World Academy for the Future of Women, Thunderbird School of International Management, and UCP-SARnet. I understand that this program can present circumstances and conditions not always found within my own country or geographic area, and may be a risky and potentially dangerous activity to me. Consequently, I assume all risk, which may be associated with this program. Further, in consideration for me being allowed to participate I release and hold harmless the organizers named above from all risks attendant thereto and agree as follows:
1. I understand that rarely, but from time to time, an emergency may develop which necessitates the immediate administration to me of medical care, hospitalization, or surgery. Therefore, in the event of my injury or illness, when I am unable to do so for myself, I authorize Sias International University, or my University representative, if present, by and through its authorized representative(s) who are in attendance at the International Leadership Summit and in charge of the study program in which I am a participant, to secure and agree to any necessary medical treatment for me, including the administration of an anesthetic and surgery. I agree and understand that any such medical treatment is solely at my expense. I also agree to indemnify and reimburse Sias International University for any loss or expense, which it might suffer on account of my injury or illness and any treatment rendered therefore. I certify that I presently have health and hospitalization insurance which will cover me when I am participating in the International Leadership Summit and which I will keep in force during the time that I participate in the Summit.
2. In consideration of being permitted to participate in the International Leadership Summit and fully understanding that such participation may include the risk of serious personal injury, I fully and absolutely assume full responsibility for the risk of injury due to such participation of any magnitude, including fatality, and do hereby forever absolutely release and agree to hold harmless Sias International University and the organizers. I also understand that claims may be made by other persons against Sias International University and the organizers as a result of any injuries or illnesses caused by or to me and further agree to indemnify, defend and hold Sias International University and the organizers harmless from any judgment, legal expense, or other expense of any nature which Sias International University and the organizers may incur as a result of defending any such claim for injury, illness, or damage.
3. I understand that as an American citizen or foreign national, in a foreign country, that I will be subject to all the laws of the host country. I agree to familiarize myself with the laws of the host country in which I will be studying before I visit. While a guest in China and on Sias International University campus in which I am studying, I agree to conduct myself in a manner that will bring credit to my University, the organizers and to me. I agree that I will comply with all the laws of the host country, the regulations of Sias International University and the organizers and its programs. I understand that, in addition to the imposition of fines or penalties imposed by the country in which I am studying, my inappropriate or illegal conduct while in that country may, at the sole judgment of Sias International University and the organizers, cause the discontinuance of my participation in the International Leadership Summit.
4. I understand that because I am solely responsible for my conduct, if my acts of either omission or commission, cause Sias International University or the organizers to expend any monies or incur any loss as a result of my conduct, that I will reimburse Sias International University or the organizers for any and all expenses incurred. Further, I agree that I am responsible for any and all costs to Sias International University and organizers arising from my voluntary or involuntary withdrawal from the International Leadership Summit prior to its completion, including any withdrawal caused or contributed to by an illness, injury, or disciplinary action.
5. The undersigned understands that the execution of this General Release and Hold Harmless Agreement is a requirement of being allowed participating in the International Leadership Summit.
